A form called RP04 will need to be completed and re-submitted to CH. The first part of the form is to identify the inaccurate information and the second part to effectively offer a second filing with the accurate information on it. The form and all relevant guidance notes can be found here https://www.gov.uk/government/publications/file-a-second-filing-of-a-document-previously-delivered-rp04
